The Boy’s Friendship Foundation (BFF) was a charitable organization operating in Rotborough, Missouri with the stated mission of supporting children recovering from serious illnesses, created in the name of Boy the Cow and operated at least in some capacity by ROTCAN, who broadcast Boy's Friendship Ranch and largely managed Boy's media presence as a local celebrity.
History
According to promotional material published on the My Pet Boy website, BFF primarily focused on aiding local children experiencing loneliness, bullying, and other social difficulties during hospital stays. The foundation claimed to offer emotional support and community-based outreach, positioning itself as a supplemental care provider for families receiving treatment at Saint Mary's Baptist Holy Redemption Regional Medical Health Memorial Children’s Hospital.
Over time, BFF expanded its public programming to include after-school services, volunteer opportunities, and local fundraising events. Notably, the foundation was sponsored by NXHealth, Growing Minds Learning Center, and the Kidz 4 Mayor Gray After School Initiative, all of which were tied in various ways to Rotborough’s private and municipal sectors. BFF’s most visible contribution involved the reported donation of RageBoy consoles and school supplies to pediatric care units, in partnership with the game publisher Rotplay during the promotional cycle for My Pet Boy. Part of this initiative included a retail release of Boy the Cow plushies in collaboration with RotPlay's parent company, RotToys.

Controversy & Shutdown
In February 2005, the Boy's Friendship Foundation (BFF) was shut down following an investigation into the deaths of several children at Saint Mary's Baptist Holy Redemption Regional Medical Health Memorial Children’s Hospital. Authorities discovered that a volunteer, Michael Baylez, had uploaded videos and photographs of himself tampering with patients’ oxygen machines and intravenous lines. The material was circulated online, leading to Baylez’s dismissal and subsequent arrest. The incident caused significant reputational damage to the foundation, prompting sponsors and investors to withdraw support. As a result, the organization declared bankruptcy shortly after the scandal.
